If your doctor has told you to take iron tablets, you are not alone. According to the National Family Health Survey (NFHS-5), 57% of Indian women aged 15 to 49 are anemic. That means more than half of Indian women do not have enough iron in their blood.
Iron tablets are one of the most commonly prescribed supplements in India. But here is the problem: many women take them the wrong way, so their body absorbs very little of the iron. This article will help you understand why Indian women are so prone to anemia, and how to take your iron tablets correctly so they actually work.
Why Are So Many Indian Women Anemic?
There are three main reasons why anemia is so common among Indian women:
- Heavy menstrual periods: Women lose iron every month during their period. Women with heavy bleeding lose even more iron, and the body cannot always make up for it through food alone.
- Vegetarian diet: India has a large vegetarian population. Plant-based iron (called non-heme iron) is harder for the body to absorb compared to iron from meat (heme iron). Even women who eat well may not absorb enough iron from a purely vegetarian diet.
- Tea and coffee with meals: This is a very common Indian habit. Drinking chai or coffee within 1 to 2 hours of a meal blocks your body from absorbing iron from that meal. The tannins in tea bind to the iron and carry it out of the body.
The #1 Mistake: Drinking Chai or Coffee Near Your Iron Tablet
This is the single biggest mistake Indian women make with iron supplements. Studies show that drinking tea or coffee within 2 hours of taking an iron tablet can block up to 60% of the iron absorption.
Think about that. If your doctor prescribed 60 mg of iron, your body may only absorb 24 mg because of your morning chai. That is a huge loss.
The Best Way to Take Iron Tablets
For the best absorption, follow these tips:
- Take iron on an empty stomach — ideally 1 hour before breakfast or 2 hours after a meal. This gives the best absorption because there is nothing in your stomach to block the iron.
- Take it with vitamin C — Vitamin C helps your body absorb iron much better. Eat an amla (Indian gooseberry), drink a glass of lemon water, or have an orange along with your iron tablet. Even a small amount of vitamin C makes a big difference.
- Use plain water — Swallow the tablet with a full glass of plain water. Not milk, not chai, not juice (unless it is a citrus juice like orange or lemon).
What NOT to Take With Iron
Some foods and medicines block iron absorption. Avoid taking these at the same time as your iron tablet:
- Milk, curd, paneer, or cheese — Calcium in dairy products blocks iron absorption. If you take a calcium supplement, space it at least 2 hours apart from your iron tablet.
- Antacids (like Digene, Gelusil) — These reduce stomach acid, which you need to absorb iron.
- Tea, coffee, green tea — Tannins in these drinks bind to iron and prevent absorption.
- Whole grains and legumes at the same time — Phytates in these foods reduce iron absorption. This does not mean you should stop eating them, just do not take your iron tablet along with a heavy dal-roti meal.
Iron and Thyroid Medicine: Keep Them 4 Hours Apart
Many Indian women take thyroid medicine (levothyroxine) along with iron supplements. This is a common combination, but you must be careful.
Iron blocks the absorption of thyroid medicine. If you take both together, your thyroid medicine will not work properly. Always keep at least 4 hours between your iron tablet and your thyroid medicine.
A simple routine: take thyroid medicine first thing in the morning on an empty stomach. Take your iron tablet in the afternoon or evening, at least 4 hours later.
Side Effects: What Is Normal
Iron tablets have some common side effects. These are normal and usually not a reason to stop taking them:
- Constipation — This is the most common side effect. Drink plenty of water, eat fruits and vegetables, and stay active. If constipation is severe, ask your doctor about a stool softener.
- Dark or black stools — This is completely normal. Iron changes the color of your stool. It does not mean there is blood in your stool.
- Nausea or stomach upset — Some women feel queasy after taking iron on an empty stomach.
Get Ferritin Checked, Not Just Hemoglobin
Most doctors check your hemoglobin level to see if you are anemic. But hemoglobin alone does not tell the full story.
Ferritin is the protein that stores iron in your body. Think of hemoglobin as the money in your wallet and ferritin as the money in your bank account. You can have a normal hemoglobin level but very low ferritin, which means your iron stores are almost empty. You may still feel tired, have hair fall, or feel weak.
Ask your doctor to check your serum ferritin level along with your hemoglobin. A ferritin level below 30 ng/mL usually means you need iron supplements, even if your hemoglobin looks okay.
How Long Should You Take Iron Tablets?
This is where many women make another mistake. They stop taking iron as soon as their hemoglobin reaches a normal level. But your iron stores (ferritin) take longer to fill up.
The general guideline is: continue iron tablets for 3 to 6 months after your hemoglobin reaches a normal level. This gives your body enough time to rebuild its iron stores. Your doctor will tell you the exact duration based on your blood reports.
Do not stop your iron tablets on your own. Always check with your doctor before stopping.
A Simple Daily Routine for Iron Tablets
- Wake up, take thyroid medicine (if prescribed) with plain water.
- Wait 30 to 60 minutes, have breakfast. No chai yet.
- Wait 1 hour after breakfast, then have your chai or coffee.
- In the afternoon (at least 4 hours after thyroid medicine), take your iron tablet with lemon water or amla on an empty stomach.
- Wait 1 hour before your next meal or tea.
